呼吸与危重症医学科

暨南大学附属第一医院呼吸与危重症医学科拥有一支技术精良的医疗队伍,包括教授/主任医师3名,副主任医师3名,主治医师4名,其中具有博士学位的5人,硕士学位的6人,科室已经培养硕士研究生七十多人。

科室拥有病床48张,其中呼吸重症监护室(RICU)病床6张。设有支气管镜室、肺功能室及呼吸专科门诊。拥有多种先进仪器设备,包括美国先进的有创呼吸机6台和无创呼吸机10台,惠普多功能监护仪12台,电子及纤维支气管镜2套及全套图文处理系统,德国JAEGER公司高档肺功能仪等。

长期以来,科室以医院的整体设施为平台,充分利用医院影像中心、细菌室、血气分析室、病理科等科室的条件,紧密结合临床,围绕支气管哮喘、慢性阻塞性肺疾病(COPD)、呼吸衰竭、肺部感染、慢性咳嗽、肺癌、胸膜疾病、肺栓塞、肺间质性疾病及睡眠呼吸障碍性疾病的发病机制与防治等开展了大量的临床,教学,科研工作,并取得了明显的成绩。

科室在治疗急、慢性呼吸衰竭方面积累了丰富的经验。大力开展早期无创通气、有创无创通气序贯,结合药物和辅助物理治疗,使呼吸衰竭的抢救成功率明显提高。近十年来,大力拓展无创通气技术的应用范围,如各种原因所导致的缺氧和呼吸困难、急慢性呼吸衰竭、哮喘持续状态、中重度肺炎、外科术后合并呼吸困难、阻塞性睡眠呼吸障碍综合症、急慢性心功能不全等疾病,使既往需要气管插管或气管切开的病人通过无创通气治疗转危为安,大大减少了创伤和医疗费用,收到了良好效果。

微创技术(经中心静脉导管)治疗自发性气胸是科室在本地区率先倡导使用的一项新技术,与传统胸腔闭式引流术相比,大大减少创伤和风险,缩短了住院时间,提高了治愈率。此项技术作为科室的常规治疗手段,已使众多患者受益。通过胸腔局部化疗和胸膜粘连相结合的方法,提高了晚期恶性胸腔积液患者的生命质量。

科室擅长经支气管介入治疗和“无痛”支气管纤维镜诊疗技术,开展常规纤支镜检查,取活检、刷检,防污染毛刷取标本,经支气管肺活检,经支气管针吸活检、支气管肺泡灌洗,提高了气道内疾病及恶性肿瘤的诊断和分期水平;经纤支镜介入治疗气道狭窄,包括激光、电烧灼、气道内支架植入、球囊扩张、冷冻及药物注射等,提高了气道狭窄患者的救治水平;异物摘取,经纤支镜引导气管插管、胃管插管等拓宽了纤支镜的应用范围。

肺功能检查是呼吸系统疾病的必要检查,对于早期检出肺、气道病变,评估疾病的病情严重程度及预后,评定药物或其它治疗方法的疗效,鉴别呼吸困难的原因,诊断病变部位、评估肺功能对手术的耐受力或劳动强度耐受力及对危重病人的监护等方面有重要的指导意义。科室在原有肺功能仪的基础上,新引进了德国JAEGER公司Master Sereen PFT肺功能仪,可以开展常规(普通)肺功能、肺容量检查、支气管舒张试验、支气管激发试验、弥散功能及残气容积检查。

科室曾主持和参加国家、省、部、市级课题的相关研究,如支气管哮喘的基础与临床研究涉及国内过敏原的理化性状的研究和哮喘神经源性气道炎症机制、咳嗽变异型哮喘(慢性咳嗽)、慢性阻塞性肺疾病的基础和临床研究等,并在国内、外学术期刊上发表论文一百余篇。

经过全科医护人员的不懈努力,科室不断发展壮大,形成了自己的医疗及科研特色。目前已成为在省内呼吸界享有一定声誉,基础设施和人员素质以及诊疗水平居全省先进行列的科室。

The department of pulmonary medicine is a professional unit which possesses a number of qualified doctors with responsible and experienced echelon, including four professor/chief physician, three associate chief physicians and two attending physicians. In this echelon, three own doctor degree; six own master degree and more than sixty postgraduates have enrolled in our department master programs.

We have total 48 patient beds, including 6 beds respiratory intensive care unit in our department. In addition, our department owns many kinds of facilities for diagnosis and treatment such as invasive and non-invasive ventilators, multifunction monitors, fibreoptic bronchoscopes as well as pulmometers.

Our hospital has the advanced medical equipments and complete infrastructures.Utilization of the advanced resources within the imaging center, bacteria room, blood gas analysis room and the department of clinical pathology, we have advantages in academics,scientific research on the pathogenesis, prevention and the cure of various respiratory infectious diseases, COPD, allergic diseases, acute and chronic respiratory failure, lung cancer, pleural diseases and other difficult and critically cases.

We are experience in the therapy of acute and chronic respiratory failure by carry out the early non-invasive positive pressure ventilation (NIPPV) ,remedy and physical therapeutics. The rate of successful rescue are improved obviously. In the past 10 years, the application scope of the technique of non-invasive mechanical ventilation has developed, such as hypooxia and dyspnoea brought about by any cause, acute and chronic respiratory failure, status asthmatics, medial to severe pneumonia, severe anemia, obstructive Sleep Apnea Syndrome(OSAS) as well as acute and chronic cardiac insufficiency, etc. Not only had the non-invasive mechanical ventilation technique been able to advert a danger that used to require a tracheal intubation or incision of trachea, but also reduced the trauma and medical cost geatly.

With our great efforts, the department has the ascendancy over other hospitals in such academic area as minimally invasive technique (transvenous catheter) to cure spontaneous pneumothorax, which is at the leading place in Guangdong province. Compared to the traditional therapy, the pleural closed drainage has a serial of good points: the trauma and the risk enormously decreased, length of stay shortened, the healing rate increased. Up to now, this technique has been regarded as a conventional therapy. A great many patients have taken advantage of this excellent technique. Through the combination of local chemotherapy in thoracic cavity and pleural adhesions, the quality of life with malignant pleural effusion patients are enhanced.

Meanwhile, our department is also adept at diagnosing and treating various respiratory diseases through transbronchial interventional therapy and aponia technique by the techniquefibreoptic bronchoscopy. The indications involve foreign body of bronchi, hemoptysis of unknown origin, lymphadenectasis, protracted cough and expectoration, bronchial asthma, common disease as well as frequently encountered disease. The technique provides advantage for a great many patients.

Pulmonary function tests are necessary in the examination of respiratory diseases, especially for the early detection of lung and airway disease. Moreover, the test can assess the severity and prognosis of the disease, the effectiveness of drug or other treatment, and identify the causes of dyspnea. Also, it can make evaluation on the pulmonary function of a patient before the operation and play an important role in monitoring the critical patient. Based on the original instrument, we also measured the pulmonary function, lung capacity, bronchial dilation, bronchial provocation test ,diffusing capacity and the residual gas volume by using a computerized system, Master Screen PFT,which is from Jaeger, Germany.

We have ever hosted and participated many scientific research works. The projects undertaken include The Basic and Clinic Research about Bronchial Asthma, which is referred to the study on the physical-chemical properties of the domestic sensitinogen and the mechanism of the neurogenic airway inflammation of asthma; Cough Variation Asthma (Chronic Cough); chronic obstructive pulmonary disease, and so on. For the past ten years, there have been more than thirty pieces of clinical research articles have been published in the SCI and core periodicals such as THORAX, RESPIOLOGY and《Chinese Journal of Tuberculosis and Respiratory Diseases》.

Today, the department of pulmonary medicine has an excellent reputations in the province. Its progress has been possible given the continous hard work physicians who own the superb clinical experience, the strong patience and love. We will ensure that the clinic will be improved continually, we will do our best.
